People living with diabetes must manage every day to care for themselves and their families while also managing their chronic illness, along with managing their role and emotional management and must undertake certain tasks to live well with diabetes. Self-Management support offers people living with diabetes the education and supportive interactions so that they may gain the skills and confidence in continuing to manage well.

The Living Healthy Champlain offers the Stanford model peer-led chronic disease illness workshops, Living a healthy life with chronic conditions, for those living with or caring for people living with chronic illness. Workshops are offered across the region in a diversity of languages and times.
